Semi-private or private room accommodations. ·
Meals and other nourishment, including special
diets, as ordered by the physician. ·
24 Hour Nursing care. · Assistance with activities of daily living. ·
Personal care items –
standard stock soap, shampoo, non-medicated lotions/powders. Anything other than standard stock items
must be purchased from the resident’s personal funds. ·
Laundry services for personal clothing. ·
Oxygen concentrators
or tanks. ·
Prescribed medical
dressing items. ·
Social Services. ·
Dental evaluations and
routine dental treatment. ·
Recreational
activities (with the exception of luncheon trips, personal shopping trips or
other function requiring an admission fee or cost, will require the resident
to use their personal funds). Our daily room rate does not include: ·
Physical, Occupational and Speech therapies, as
ordered by a physician. ·
Physician services
will be billed to resident’s health insurance carrier. Any portion not covered by insurance
benefits will be billed to the resident or responsible party. ·
Podiatry services will
be billed to the resident’s health insurance carrier. Any portion not covered by insurance
benefits will be billed to the resident or responsible party. ·
Pharmacy services
(drug and special medical treatment items) will be billed to you by Unicare
Pharmacy Services. ·
Beautician/Barber
services. ·
Medical supplies such
as catheters may be billed to the resident if Medicare Part B is not carried. ·
Repairs to any personal medical equipment
(wheelchairs, hearing aids, glasses, etc.) or supply replacements (batteries
for electric wheelchairs, hearing aids, etc.). |
Dear
Applicant/Responsible Party: Thank
you for your interest in Sullivan County Health Care (SCHC). Our home is Medicaid licensed for 156 beds
as an Intermediate Care Facility (ICF) and is certified by the New Hampshire
Bureau of Health Facilities Administration.
The nursing home provides 24-hour Nursing care and other services to
residents whose health care needs cannot be successfully managed at home or
in a community setting. Sullivan
County Health Care is a Medicare approved Skilled Nursing Facility
(SNF). If your doctor determines that
Skilled-nursing placement is appropriate, following a qualifying stay in the
hospital, we are able to accommodate your needs. Once
the facility application is completed in full and returned, medical
information will be obtained to assist in determining nursing home
placement. If necessary, a home visit
will be scheduled with our External Care Coordinator. If an appropriate bed is not available, a
waiting list is maintained, with Sullivan County residents having first
priority. If an applicant has limited assets, an application for Medicaid must be made prior to admission through the Division of Health & Human Services in Claremont (17 Water Street, 603-542-9544). YOU MUST CALL MEDICAID FOR AN APPOINTMENT; PLEASE DO NOT JUST STOP BY. Spouses of applicants should consider having a resource assessment done by them in order to determine how and when assets should be divided. In many cases, the community spouse may be able to keep a portion of the applicant’s income to meet their own expenses. If you are a Medicaid recipient, or will be applying for Medicaid, an assessment will need to be done prior to admission for the state to determine eligibility for nursing home level of care. We will coordinate the necessary paperwork for this assessment with your doctor and/or home health agency. As
